About Our Practice

As your Medical Home, we will:
  • Provide you with a care team who will know you and your family.
  • Discuss your goals and how you would like to improve your health.
  • Respect you as an individual. We will not make judgements based on race, religion, sex, age, disability, etc.
  • Respect your privacy. Your medical information will not be shared with anyone unless you give us permission
    or it is required by law.
  • Help you stay healthy by giving you easy to understand information.
  • Give care that meets your needs and fits with your goals and values; care that is based on quality and safety.
  • Respond promptly to your calls, questions and concerns.
  • Have a doctor on call after hours for your urgent needs.
  • Remind you when vaccines and tests are due.
  • Notify you of test results in a timely manner.
  • Take care of acute illnesses, chronic diseases and give you advice to help you stay healthy.
  • Help coordinate care with speciality doctors if needed.
  • To improve your care we are using technology including our Electronic Medical Record,
    and we will strive to continuously improve.
As your Medical Home, we trust YOU to:
  • Tell us what you know about your health and illnesses. Tell us about your needs and concerns.
  • Take part in planning your health care.
  • Follow the care plan that is agreed upon as best you can.
  • Learn about health and wellness and how you can help prevent disease.
  • Tell us about all medications and over-the-counter supplements you are taking.
  • Ask for refills at your office visit, and not call for refill requests by phone.
  • Let us know when you see other health care providers and ask them to send us a report about your care.
  • Inform us of any change that is made in your medication or treatment.
  • Keep appointments or call to reschedule or cancel in a timely fashion.
  • Call or email if you do not receive your test results within 2 weeks.
  • Use the after-hours line only for issues that cannot wait until the next work day.
  • Learn about your insurance so you know what it covers and what is does not, and work with us to help
    develop a payment plan. Pay your share of the visit fee when you are seen in the office.
  • Respect us as individuals and partners in your care and give us feedback to help improve our services.
We provide a variety of services for the entire family, including:
  • Physical exams. Wellness and Health Maintenance Care. Pap Smears, Prostate exams.
  • Well child exams and immunizations.
  • Chronic Disease Management
  • Urgent care and sick visits, including minor emergencies
  • Osteopathic Manipulative Medicine
  • Group visits for chronic medical conditions
  • Internet access to providers in secure manner
  • Written care plans are available

Office Hours: 
Monday - Thursday 8:00am-5:00pm, Friday 8:00am-3:00pm.
After-hours answering service is available for emergencies only.
Medication refills are not done over the phone, but rather, at appointments only.

Appointments:
We prefer to see patients by appointment. We also have many appointments for same-day sick calls,
available each day on a first come first serve basis.
Call (517) 913-3980 for any appointments.

Cancellation policy:
If you cannot keep an appointment, please call the office to notify us immediately.
If you fail to show up for an appointment, or do not give us at least a 4 hour notice, you will be charged a fee.
($25 for standard appointments and $75 for extensive appointments/physicals)

Payment of Services:
Most plans include a co-pay or deductible. These fees are due at the time of service. If we are not a covered provider,
of if the service is not a covered benefit for your insurance plan, it is your responsibility to make payment of services.

Insurances:
We participate with many insurance plans: including most BCBS, BCN, PHP, SPHN, Medicare, Cigna, Aetna, Tricare, Cofinity

Hospitalization:
Our health care providers do not administer care in the hospital, but rather rely on a Hospitalist Physician at each of the area hospitals. This is a physician whose specialty is hospital care, and they communicate with us regarding any admissions of our patients. Because their focus is in-patient care this supports shorter hospital stays.

Urgent Care:
Our practice strives to take care of patients who need urgent or same day care. Please call us to see if we can see you in the office, or guide your care. If you need to go to an Urgent Care or Emergency Room, we can inform them of your health history. The following urgent care centers have agreed to provide Lake Lansing Family Practice with a prompt summary of your urgent care treatment and we encourage you to use them:
